I've been told a few people are having problems with PB not updating and kicking then into spec mode.
To fix the majority of problems Click Here (http://www.evenbalance.com/index.php?page=updatepb.php) and download PBWEB. Place it in your C:\Program Files\Quake III Arena\pb\ folder (amend this to match your install location) and then run it.
If you have any other problems please let me know by posting below and I will look into it and see if I can find a fix.
